| 内容記述 | 症例は27歳(経過観察最終時)。男性。左大腿骨骨肉腫広範切除術後に人工関節の緩みが生じ機能再建を目的に1992年4月膝rotation plastyが施行された。手術後理学療法における機能獲得の推移を後方視的に調査した。調査期間は1992年から1996年の4年間であった。患肢の関節可動域は術後20週でほぼ正常域に達したが,股関節周囲の十分な筋力の回復には20ヵ月を要した。術後14ヵ月で独歩を開始するも,骨癒合が完成するまでに約2年を要し,その間は坐骨支持つき長下肢装具を使用した。骨癒合完成後は足底のみでの荷重が可能となり,膝継手を伸展-5°から屈曲90°まで動かすことが可能となった。最終経過観察時ではスポーツ活動も可能となり,広範切除術後の機能に比べ高い活動性が獲得されていた。膝rotation plastyの術後理学療法では骨の癒合状況に応じた長期的な理学療法の介入が必要といえる。 | |||||||

| 内容記述 | The case is a 27-year-old man (at the time of the last follow-up). After a diffuse excision of an osteosarcoma in his left thigh, his artificial joint became loose, and a knee rotationplasty was performed in April 1992 to restore function. After the surgery, the transition of function acquisition in physical therapy was retrospectively investigated for four years, from 1992 to 1996. The range of joint movement of the affected limb had reached almost normal range 20 weeks after surgery, but he needed 20 months to recover enough muscle strength around the hip joint. He started walking independently 14 months after surgery, but about two years was needed for the bone adhesion to solidify. During this time, a knee-ankle-foot orthosis with an ischium support was used. After the bone adhesion was complete, weight bearing was allowed only on the sole, and the knee joint could be moved from -5° of extension to 90° of flexion. At the last follow-up, his was participating in sport activities, and he was able to perform higher functions than he did after the excision surgery. For physical therapy after after knee rotationplasty, long-term intervention must be used to accommodate bone adhesion. | |||||||
